This role is is a part time position working 60 hours/fortnight (4 days per week) and is on a contract ending June 2022 (with potential for renewal). This role will commence on August 24th.What will I be doing as a home support assessor?· Community based role, visiting older Australian’s in their homes. You will drive throughout your broader region with a rostering team determining the best run to your first client and home from your last client· Conducting holistic assessments with a focus on wellness and reablement and a client centred approach· Capturing vital client information to build on the My Aged Care client story· Developing support plans and identifying unique individual goals· Connecting people with appropriate services and supports in their community. This may include assistive technology, social support or community careNeed to be/have:· Knowledge of assessment and goal orientated support planningin aged, community, health or social services· Previous experience working in aged care, community services, assessment and/or coordination of services· Confident communication with the ability to engage and motivate clients, families, staff and other stakeholders with a positive approach· The ability to attend up to 2 weeks of full time training (all costs covered) at our head office in Coolangatta, QLD. Orientation is currently being delivered virtually due to COVID-19, subject to change.· Some local knowledge and/or the ability to network with community aged care service providers and stakeholder within your region· Located between Nowra to Milton areas· Comfortable travelling long distances by carWhat to bring?· Relevant qualifications in nursing, allied health, aged care, community services or similar· Unrestricted drivers licence and own reliable insured vehicle. Salary sacrificed vehicle can be discussed for full time positionBonus Points:· Qualified RAS Assessors are encouraged to apply· Knowledge of services available· Use of My Aged Care systems and National Screening and Assessment Form (NSAF)· Registered nurses or allied health professionals encouraged to applyAbout Aspire 4 Life:Aspire 4 Life is a leading assessment and case management agency delivering world-class assessment, planning, coordination, restorative and wellbeing services with a wellness and reablement focus.
