Home Support Assessor – Casual/Locum

The ideal candidate can reside anywhere near a major airport in QLD. You will flex across regions where there is a short term need for an additional assessor, as well as being able to reduce reliance on subcontractors.What will I be doing as a casual/locum home support assessor?· Community based role, visiting older Australian’s in their homes. You will drive or fly throughout our 17 regions across QLD and NSW with trips typically between 3 - 5 days (Monday - Friday)· Conducting holistic assessments with a focus on wellness and reablement and a client centred approach· Capturing vital client information to build on the My Aged Care client story· Developing support plans and identifying unique individual goals· Connecting people with appropriate services and supports in their community. This may include assistive technology, social support or community careNeed to be/have:· Knowledge of assessment and goal orientated support planning in aged, community, health or social services· Previous experience working in aged care, community services, assessment and/or coordination of services· Confident communication with the ability to engage and motivate clients, families, staff and other stakeholders with a positive approach· The ability to attend up to 2 weeks of full time training (all costs covered) at our head office in Coolangatta, QLD. Orientation is currently being delivered virtually due to COVID-19, subject to change.· The ability to network with community aged care service providers and stakeholder within your region· Ability to travel by car or plane for days at a time to conduct assessments as required· Located nearby to a major airport in the state of QLDWhat to bring?· Relevant qualifications in nursing, allied health, aged care, community services or similar· Unrestricted drivers licence and own reliable insured vehicle.· Ability to travel at minimal notice and flex as needed for the work on offerBonus Points:· Qualified RAS Assessors are encouraged to apply· Knowledge of services available· Use of My Aged Care systems and National Screening and Assessment Form (NSAF)· Registered nurses or allied health professionals encouraged to applyAbout Aspire 4 Life:Aspire 4 Life is a leading assessment and case management agency delivering world-class assessment, planning, coordination, restorative and wellbeing services with a wellness and reablement focus.
