We have an exciting opportunity to join the North Sydney Regional Assessment Service with Aspire4Life as a Home Support Assessor. In this role, you will conduct assessments using a holistic and client centered approach, with a focus on wellness and reablement. These assessments will support older Australians to continue living independently within their chosen community, by considering needs such as assistive technology, social support services, and community care. You will have the opportunity to travel throughout the North Sydney Aged Care Planning region (Northern Beaches/Ryde/Killara areas) approx 30km from home, to work collaboratively with clients in their homes. This position will commence at the end of June 2021, on a fixed term contract until 30 June 2022 (with the possibility for renewal). Candidates will ideally reside in or nearby to the Chatswood/Ryde suburbs. This position offers a fantastic 2 weeks of full-time training and orientation at our head office in Coolangatta, QLD (all expenses covered), commencing end of June 2021. You can also take advantage of other benefits, including salary and entertainment packaging and discounted gym attendance for applicable gyms.A typical week as an assessor: Travel throughout the North Sydney Aged Care planning region region, visiting clients in their homes as you utilizie your clinical expertise to conduct holistic style assessments, with a focus on wellness and reablementCapture vital client information to build on the My Aged Care client story and develop an individualised support plan underpinned by the clients unique goalsDevelop strategies to connect people with appropriate services and supports in their community (may it include assistive technology, social support or community care) to further empower the client with their independenceNeed to be/have:Knowledge of assessment and goal orientated support planning in aged, community, health or social services, and previous experience working in aged care, community services, assessment and/or coordination of servicesConfident communication and interpersonal skills, with the ability to engage and motivate clients, families, staff and other stakeholders with a positive approachSome local knowledge and/or the ability to network with community aged care service providers and stakeholder within your regionWhat to bring?Tertiary qualification as a Registered Nurse, Social Worker, Occupational Therapist or Physiotherapist (very highly regarded)Current AHPRA Registration (or relevant)Unrestricted Australian driver's licence and own reliable insured vehicle (and be comfortable travelling long distances by car)You will be supported by our vibrant centralised liaison team who coordinate directly with the clients – you will not need to schedule appointments or worry about planning travel distances or timings!
