RAS (Regional Assessment Service) Assessor – Southern Highlands Region



As a Home Support Assessor you will utilise a number of technologies, including the National Screening and Assessment Form, video conferencing, internal social media together with various software products, to support clients and stay connected with your team.  This is a community based role which will see you visiting our client’s in their own homes, with potential travel to Queanbeyan, Young, Goulburn, Cooma and Jindabyne. Please note that overnight stay may be required one night per fortnight.Ideal applicants will be located in or around the Canberra area or further south either inland or along the coast (approx. to Eden).What will I be doing as a home support assessor?Community based role, visiting older Australian’s in their homesConducting holistic assessments with a focus on wellness and reablementCapturing vital client information to build on the My Aged Care client storyDeveloping support plans and identifying unique individual goalsConnecting people with appropriate services and supports in their communityNeed to be/have:Knowledge of assessment and goal orientated support planning in aged, community, health or social servicesGeneral knowledge of CHSP Previous experience working in aged care or community servicesConfident communication with the ability to engage and motivate clients, families, staff and other stakeholders with a positive approachThe ability to attend up to 2 weeks of full time training (all costs covered) at our head office in Coolangatta, QLD.Some local knowledge and/or the ability to network with community aged care service providers and stakeholder within your regionComfortable travelling long distances by car  What to bring?Relevant qualifications in nursing, allied health, aged care, community services or similarUnrestricted drivers licence and own reliable insured vehicle Brownie Points:Qualified RAS Assessors are encouraged to applyUse of My Aged Care systems and National Screening and Assessment Form (NSAF) About aspire4life:aspire4Life, powered by Feros Care, is a leading assessment and case management agency delivering world-class assessment, planning, coordination, restorative and wellbeing services.
