RAS (Regional Assessment Service) Assessor – Darling Downs Region



As a Home Support Assessor you will utilise a number of technologies, including the National Screening and Assessment Form, video conferencing, internal social media together with various software products, to support clients and stay connected with your team.  This is a community based role which will see you visiting our client’s in their own homes. Please note that overnight stay may be required one night per fortnight and the role itself will entail a lot of travel in between clients on a daily basis.   • Help coordinate care and support for elderly clients
  • Mobile role, covering the Darling Downs region
  • Part time position (60 hrs/fortnight) on a 12 month contract

What will I be doing as a home support assessor?

  • Developing support plans and identifying unique individual goals
  • Connecting people with appropriate services and supports in their community

Need to be/have:

  • Knowledge of assessment and goal orientated support planning in aged, community, health or social services
  • General knowledge of CHSP 
  • Previous experience working in aged care or community services
  • Confident communication with the ability to engage and motivate clients, families, staff and other stakeholders with a positive approach
  • The ability to attend up to 2 weeks of full time training (all costs covered) at our head office in Coolangatta, QLD.
  • Some local knowledge and/or the ability to network with community aged care service providers and stakeholder within your region
  • Located in the Darling Downs region 
  • Comfortable travelling long distances by car
