RAS (Regional Assessment Service) Assessor – Northern Sydney Region



As a Home Support Assessor you will utilise a number of technologies, including the National Screening and Assessment Form, video conferencing, internal social media together with various software products, to support clients and stay connected with your team.  You will travel extensively across your region to conduct assessments in various areas and geographic locations. The majority of assessments will be done face to face, with your ability to build rapport second to none.What will I be doing as a home support assessor?Community based role, visiting older Australian’s in their homesConducting holistic assessments with a focus on wellness and reablementCapturing vital client information to build on the My Aged Care client storyDeveloping support plans and identifying unique individual goalsConnecting people with appropriate services and supports in their communityNeed to be/have:Knowledge of assessment and goal orientated support planning in aged, community, health or social servicesGeneral knowledge of CHSP Confident communication with the ability to engage and motivate clients, families, staff and other stakeholders with a positive approachThe ability to attend up to 2 weeks of full time training (all costs covered) at our head office in Coolangatta, QLD.Some local knowledge and/or the ability to network with community aged care service providers and stakeholder within your regionLocated in Sydney area, in the North Eastern region (ie.
