Telehealth Coordinator



Full Time, 2 Year Fixed Term Contract Company Vehicle, Laptop & Smart Phone ProvidedExciting Newly Created Role in Our Telehealth Program We are currently seeking a Registered Nurse or Allied Health professional with experience in business development to join our new Telehealth initiative in Central and East Sydney. The goal of our Telehealth service is to help seniors over 70 years build their knowledge and skills in self-management of their chronic disease, working closely with their GP and other supports.About the Role:As the key clinician and face of this new initiative, you will develop strong relationships with the local primary health network and promote Feros Care and our Telehealth Service. You will also provide some face to face support to clients in the Central and East Sydney regions who participate in the Telehealth service.  The position is a mobile role with visits to GP surgeries, health partners and client’s homes each week.Key Responsibilities Include:Developing positive and productive partnerships with local GP practices and proactively networking with local service providers and key referral agenciesActing as an Ambassador for Feros Care, participating in community events and initiatives to build the Feros Care brand and promote our servicesInstallation, training and ongoing support to clients in the use of Telehealth and emerging technologiesVisiting our clients in their home to conduct holistic assessments and produce goal driven support and wellness plansAbout You:Current Registration with the Australian Health Practitioner Regulation Agency (AHPRA)Demonstrated ability to develop robust and sustainable working relationships with GP’s, referral agencies and health servicesHighly developed skills in the use of smartphones, laptops, video conferencing and web access to systems for information, data entry and communicationStrong knowledge and experience in contemporary chronic disease coaching, health literacy and case managementAbout Feros Care:Feros Care is a multi-award winning not-for-profit organisation offering a range of aged, disability and community care services.
