Veterans Home Care Assessor – Tweed Heads



We are expanding! This role has been created to support our new, dynamic team within Assessment and Planning. As an assessor you will be responsible for conducting assessments and co-ordinating services to eligible veterans and war widows/widowers through Veterans’ Home Care (VHC). Our team are national leaders in providing holistic and wellness based assessments with an individual approach that promotes independence for Senior Australians to continue to live independently in the community of their choice.This role is on a fixed term contract ending June 2021 (with view for renewal). Full time Monday - Friday.  What will I be doing as a Veterans Home Care Assessor?Handling a high volume of inbound/outbound phone callsMeeting deadlines and KPIs within a busy contact centre environmentBuilding strong and genuine relationships with our clients over the phoneAccurately capturing and recording key client informationWorking within a multidisciplinary team to establish best outcomes for each clientWorking together with the client to develop a support plan and identify their individual unique goals, assisting them to connect with appropriate services and supports within their local community Need to be/have:You will have experience in understanding, developing, coordinating and evaluating client centred care, working within a multidisciplinary team and strong problem solving skills. You will be someone who is able to capture critical information quickly and accurately. You will have a strong flair for providing superior customer service and personalising our clients experience through building quick connections and identifying their current and future needs.A confident and skilled communicator with the ability to engage and motivate clients, families, staff and other stakeholders with a positive approach.
