More fulfilled.What is the Role?Our Case Managers (Wellbeing Managers) play a critical part in assisting our clients to lead independent, healthy lives by undertaking holistic evidence-based assessments, goal-driven support planning and case management.In this role you will be visiting clients’ in their homes within the beautiful Illawarra region, working in partnership, you will identify goals and create a support plan, underpinned by wellness and reablement, to assist clients to achieve their individual goals.
