We have been offering quality care and support services for older Australians since 1990, with over 700 personnel working throughout QLD, NSW, VIC, ACT and TAS.More about you, you must have:Previous experience with Home Care Packages (HCP) or Regional Assessment Service (RAS) is highly desirable or a strong understanding of HCP and RAS at a minimum.Over 3 year’s experience in case management, managing individual budgets and attending assessments with clients that include support planning and goal setting in aged, community, health or social services.Experience working remotely with a high degree of drive and be part of a wider mobile virtual team.Be confident and skilled communicator, able to engage and motivate clients, families, staff and other stakeholders with a positive approach.Have proven written and verbal communication skills including preparing reports, contributing to operational plans, conducting and facilitating presentations.Proven ability in networking and developing working relationships with referral agencies, health services and community groups.The ideal candidate will be a Registered Nurse, Allied Health Professional such as a Physiotherapist, OT, or hold a relevant certificate in Aged Care/Community/Case Management.Essential requirements of this position areThe ability to attend 10 days of initial training and orientation in our head office at Coolangatta, QLD, and 10 days of training from home (4 weeks of full time training all paid for). Hold an unrestricted Australian driver’s licence.Have a stable internet access (as you will be a virtual worker).Ability to travel interstate and stay away from home at least 2 days every month.The Wellbeing Manager is a community based role where you will travel to clients home within ACT, predominately in Canberra, to complete assessments and care plans.
