Clinical Case Manager – ACT

Join our award-winning tribe of kindness warriors as one of our Wellbeing Managers, and apply your clinical skillset to case manage your own portfolio of clients through their wellbeing journey. This is a full time, home-based role, offering flexible hours that suit both you and the client. You will split your time between working from home and within the community of the ACT to support your clients. This position will commence on an initial two-year fixed term contract, with the possibility of extension, and candidates will need to be located within the ACT aged care planning region. This is a full-time opportunity working Monday to Friday (no weekend work!), with the opportunity to utilize a company fleet vehicle for both work and personal use. This role offers a comprehensive training and orientation session to support you in your new role and allow you to get to know your new, supportive team.The Role: This is an exciting opportunity to be working within the Short Term Restorative Care (STRC) program, which is an early intervention program that aims to reverse and or slow the functional decline in older people through the delivery of an 8 week goal-oriented, multidisciplinary and coordinated range of services. You will be in control of managing your designated case load occupancy and supporting genuinely life changing outcomes for your clients.As a Wellbeing Manager you will: Help your clients to remain at the centre of their journeyBe responsible for managing individualized budgetsDevelop care plans and coordinate a multi-disciplinary team (MDT) for the delivery of in-home therapy, services, and equipmentHave the ability to exceed client expectations and maintain excellent relationships with key stakeholders About you: You will be motivated by providing exceptional client care, confident to work in a virtual environment, be a natural team player, and have:Tertiary qualifications in Nursing or Allied Health (with relevant registration)Current Senior First Aid and CPR qualificationsCase management experience – community and aged care experience highly desiredExperience working with clients applying goal-oriented, outcome focused and holistic approachesExcellent communication and customer service skillsDemonstrated ability to develop and maintain strong networks with internal and external stakeholdersExperience working remotely with a high degree of autonomy and driveCurrent drivers licenceThe Benefits:Feros Care offers some fantastic employee benefits, including generous Salary Packaging (up to the value of $15,900 per annum) and up to $2650 per annum on meal and entertainment packaging.
