Home Support Assessor – Hunter



We have an exciting opportunity to join the Hunter Regional Assessment Service with Aspire4Life as a Clinical Home Support Assessor. In this role you will conduct assessments using a holistic and client centered approach, with a focus on wellness and reablement. These assessments will support older Australians to continue living independently within their chosen community, by considering needs such as assistive technology, social support services, and community care. You will have the opportunity to travel throughout the Hunter region to work collaboratively with clients in their homes.This position will commence at the start of November on a fixed term contract until 30 June 2022 (with the possibility of renewal based on funding), and comes with the inclusion of a fleet vehicle to use for both work and personal. This is a full time opportunity working Monday to Friday (no weekend or shift work!), and candidates will need to reside in either the Scone,  Singelton or Muswellbrook areas, but will travel throughout the entire Hunter Aged Care Planning region. We offer a fantastic 2 weeks of full-time training and orientation at our head office in Coolangatta, QLD (all expenses covered), commencing November 2021. However due to the current border restrictions, orientation may be held virtually. A typical week as an assessor: Travel throughout the Hunter Aged Care Planning region, visiting clients in their homes as you utilizie your clinical expertise to conduct holistic style assessments, with a focus on wellness and reablementCapture vital client information to build on the My Aged Care client story and develop an individualised support plan underpinned by the clients unique goalsDevelop strategies to connect people with appropriate services and supports in their community (may it include assistive technology, social support or community care) to further empower the client with their independenceYou will be supported by our vibrant centralised liaison team who coordinate directly with the clients  – you will not need to schedule appointments  or worry about planning travel distances or timings! Whilst you are out on the road, your regional lead will be there to provide support and guidance along the way.Need to be/have:Knowledge of assessment and goal orientated support planning in aged, community, health or social services, and previous experience working in aged care, community services, assessment and/or coordination of servicesConfident communication and interpersonal skills, with the ability to engage and motivate clients, families, staff and other stakeholders with a positive approachSome local knowledge and/or the ability to network with community aged care service providers and stakeholder within your regionWhat to bring?Tertiary qualification as a Registered Nurse, Social Worker, Occupational Therapist or Physiotherapist (highly regarded) or minimum 3 years experience working in the aged or community care sector in a similar role, with relevant qualifications Current AHPRA Registration, or relevant (highly regarded)Unrestricted Australian driver's licence and be comfortable travelling long distances by carThe Benefits:Aspire4Life offers some fantastic employee benefits, including Salary Packaging (up to the value of $15,900 per annum) and up to $2650 per annum on meal and entertainment packaging.
